Membership form with SEPA mandate

Your club enters name, address and fees once. You get a form to fill in by hand: membership application, direct debit mandate and privacy notice, with signature fields. The PDF is in German, for German clubs.

Applies to Germany. Deadlines, tax rates, public holidays and requirements on this page follow German law. Austria, Switzerland and other countries have different rules.

What goes into the PDF?

Your club

Your entries stay on this page only, your browser remembers nothing. The form contains no details of members: they fill those in themselves.

Fees

Name, semicolon, yearly amount in euros. The amount may be left out. At most 6 lines. They appear as boxes to tick. Without lines, a free line for the type of fee is shown.

Direct debit mandate

The club gets the number from the German central bank (Deutsche Bundesbank). Without an entry, a line stays free on the form to fill in.

14 days is the standard period in the SEPA scheme. A shorter period works if you agree it with the member.

Appears in the privacy notice as a recipient. Leave it empty if you do not report members.

Without an entry, the contact of the club from above appears in the notice.

When printing, choose “Actual size” or “100%”. The mandate only counts with the original signature of the account holder. Keep it while the collection runs, and for 14 months after the last one.

How it works
  1. 01Choose the parts, then enter name and address of the club and the types of fee.
  2. 02If you like, enter the creditor ID. The page checks the structure and the check digits.
  3. 03Download the PDF, print it, and have it filled in and signed when someone joins.

Direct debit mandate. The text follows the usual SEPA template: authorisation, instruction to the bank and the note about the refund within eight weeks. The club assigns the mandate reference. It is a line to fill in on the form and belongs in the direct debit file later. You create the file with the tool SEPA direct debit (German).

Privacy notice. It names the controller, purpose, legal basis, recipients, storage period and rights. Whether you need more, for example for photos or a newsletter, depends on the club. For that there is the consent form (German).

Limits. Only the Helvetica font: characters outside Western Europe appear as question marks, and the page tells you. A4 portrait, black on white. The form contains neither member numbers nor fee calculation. It is a template to fill in yourself and not legal advice.
